Nurse Case Manager - Utilization Review - (Mon, Tue, Sat, Sun, 12p-10p)

About the role

The Nurse Case Manager - Utilization Review will work evenings at John Peter Smith Hospital, coordinating care and service for assigned patients.

Responsibilities

• Assesses all observation status patients for observation placement.
• Initial status review and level of care placement on inpatients using approved criteria.
• Conducts initial clinical assessments and discharge planning assessments.
• Maintains active communication with the patient, family, and healthcare team.
• Collaborates with the multi-disciplinary care team for timely patient management.
• Generates referrals to the Case Management Physician Advisor.
• Serves as an educational resource for case management strategies.

Requirements

• Bachelor’s Degree in Nursing from an accredited college or university.
• 3-5 years of clinical experience and/or case management, utilization review, and/or discharge planning experience in an acute care setting.
• RN licensure through the Texas State Board of Nurse Examiners.
• Possession of ACM Certification preferred.

Qualifications

• Master’s Degree in Nursing from an accredited college or university preferred.
