About the role

The Nurse Case Manager (NCM) II reports to the Manager of Case Management and Care Transitions. Under guidance of the Nurse Case Manager Team Leader, this role functions as a member of the clinical team. This role coordinates a plan of care to achieve clinical outcomes and promotes a safe and timely discharge. This position manages the plan of care throughout the care continuum to ensure the efficient movement of the patient through the hospital system. The Nurse Case Manager II serves as a clinical expert related to discharge planning, resource utilization, and case management.

Responsibilities

  • Manages the plan of care throughout the care continuum to ensure the efficient movement of the patient through the hospital system.

Requirements

  • Bachelor's degree in Nursing from an accredited school of nursing and three years work experience as a Registered Nurse required.
  • Prior leadership experience preferred.
  • One year of Case Management experience preferred.
  • Case Management certification preferred.
  • Licensure as a registered nurse by the state of South Carolina or compact state required.
  • Certification in Case Management preferred.
  • Current American Heart Association (AHA) Basic Life Support (BLS) certification or American Red Cross BLS for Healthcare Providers certification is required.

Qualifications

  • Ability to perform job functions while standing, sitting, walking, climbing stairs, working indoors, working outside in temperature extremes, working from elevated areas, working in confined/cramped spaces, bending at the waist, twisting at the waist, squatting, performing "pinching" operations, performing gross motor activities with fingers and hands, performing firm grasping with fingers and hands, performing fine manipulation with fingers and hands, reaching overhead, performing repetitive motions with hands/wrists/elbows and shoulders, using lower extremities for balance and coordination, reaching in all directions, lifting and carrying 50 lbs. unassisted, lifting/lowering objects 50 lbs. from/to floor from/to 36 inches unassisted, lifting from 36" to overhead 25 lbs., exerting up to 50 lbs. of force, maintaining 20/40 vision, corrected, in one eye or with both eyes, determining distance/relationship between objects; depth perception, matching or discriminating between colors, maintaining hearing acuity, with correction, performing gross motor functions with frequent fine motor movements, dealing effectively with stressful situations, working rotating shifts, working overtime as required, working in a latex safe environment, maintaining tactile sensory functions, and if selected, maintaining good olfactory sensory function and being qualified physically for respirator use, initially and as required.

Pay

$X.XX per hour or $Y.YY annually, depending on experience and qualifications.

Schedule

Day shift, Monday through Friday, occasional weekends and holidays, no nights. Fair Labor Standards Act Status: Salaried.
