Nurse Case Manager - Dayton, OH
About the role
The Field Nurse Case Manager role is a field/home-based position covering Dayton, OH and surrounding areas like Cincinnati and Columbus. This role involves managing medical case management for work-related injuries, collaborating with insurance carriers, medical providers, attorneys, employers, and employees.
Responsibilities
- Assess, plan, coordinate, implement, and evaluate the medical needs of injured/disabled individuals.
- Work closely with insurance carriers, medical care providers, attorneys, employers, and employees to monitor the progress of the injured worker and report results to the employer and insurance carriers.
- Manage catastrophic cases as part of the job.
Requirements
- Current, unencumbered RN license in OH.
- Experience in case management of workers' compensation cases, preferred.
- Willingness to learn and take on catastrophic cases.
- Recommended certifications (CCM, CRRN, CDMS, CRC) preferred, to be obtained within two years of hire.

- Communication skills (written, verbal, and interpersonal).
- Ability to operate autonomously with minimal oversight.
- Skilled at patient education.
- Valid Driver’s License, and good driving history.
