About the role
We are seeking a full-time Field Nurse Case Manager to cover Elizabeth and nearby communities, including Edison, Somerville, Newark, and New Brunswick. This is a field / home-based position with travel, up to 2 hours one-way (4 hours round trip per appointment). Appointments may include visits to doctors’ offices, hospitals, and various other locations, with multiple appointments possible in one day.
The Case Manager is responsible for the medical case management of work-related injuries, including assessment, planning, coordination, implementation, and evaluation of injured/disabled individuals. You will work with insurance carriers, medical care providers, attorneys, employers, and employees, closely monitoring the progress of the injured worker and reporting results back to the employer and insurance carriers.

Qualifications
- Current, unencumbered Registered Nurse (RN) license in NJ.
- Current, unencumbered Registered Nurse license in NY, preferred; required within 60 days of hire date.
- Experience in case management of workers comp cases, preferred.
- Recommended certifications (CCM, CRRN, CDMS, CRC) preferred; REQUIRED within two years of the date of hire.
Skills
- Strong organization and timeline adherence skills.
- Proficiency with PC and technology.
- Excellent communication skills (written, verbal, and interpersonal).
- Ability to operate autonomously with minimal oversight.
- Skilled at patient education.
- Valid Driver’s License and good driving history.
