About the role
This position functions in a case management role, providing care to patients across the continuum and collaborating with multidisciplinary health care professionals and community agencies.
Responsibilities
- Manages care of patients through the health care system based on the individual's needs.
- Captures and coordinates services, health promotion and counseling, disease prevention, health education and screening, and community resources linkage for patients.
- Applies clinical criteria to monitor and facilitate in-house consultations and Purchase Referred Care (PRC) to specialists outside of the facility.
- Manages aspects of the planning process in anticipation of patient variances or transitions.
- Develops an individualized comprehensive plan of care in collaboration with physicians and multidisciplinary team.
Requirements
U.S. Citizenship is required. Selective Service Registration is required for males born after 12/31/1959. This is a designated position covered by Public Law 101-630, requiring contact or control over Indian children. Due to this requirement, the agency must ensure that persons hired for these positions have not been found guilty of or pleaded nolo contendere or guilty to certain crimes.
Qualifications
- Bachelor's degree in nursing or related medical science field from an accredited professional nursing educational program.
- One full year of professional nursing experience (equivalent to the GS-9 grade level) and possession of a diploma, associate degree, bachelor's degree, or master's degree from a professional nursing educational program.
- Successful completion of a PhD or equivalent doctoral degree from a professional nursing educational program or related medical science field.
