Nurse Care Manager (Registered Nurse)

Duties

• Affirms assessments, plans, implements, and evaluates care based on age-specific components.
• Coordinates care focused on patient education, self-management, and Veteran satisfaction throughout the care continuum.
• Fulfills responsibilities according to established policies and guidelines.
• Influences care outcomes by collaborating with interdisciplinary teams.
• Displays leadership, experience, and creative approaches to managing complex client care.
• Executes responsibilities demonstrating expertise in providing complex patient management.
• Manages patient care, care coordination, and discharge/disposition planning.
• Assists Veterans, family members, and caregivers in accessing appropriate services and options for complex cardiovascular care needs.
• Coordinates care with multiple providers across the Cardiology Hub and Spoke Network.
• Ensures services interface with VA and community programs to maintain continuity of care.
• Addresses psychosocial, nursing, and medical needs of Veterans and their families/caregivers through interdisciplinary care team participation.
• Coordinates and expedites care from diagnosis to treatment.
• Analyzes and manages laboratory and radiology results, and provides information to providers.
• Utilizes databases for population management to track patient care throughout all episodes of care.
• Evaluates care and outcomes to ensure timely and appropriate service provision.
• Advocates for holistic care through patient/family education and provider interaction, implementing an educational plan to meet Veterans' care needs.
• Collaborates in the development, implementation, evaluation, and revision of policies, procedures, and guidelines.
• Demonstrates performance and leadership that improves care for a group of Veterans.
• Provides constructive feedback to peers for improvement.
• Fosters a safe and supportive environment conducive to the professional development of healthcare professionals.
• Evaluates outcomes of evidence-based decisions and practice changes for individuals, groups, and populations.
• Documents broad and complex outcomes at the program or service level that can be demonstrated at any organizational level within a facility.

Qualifications

• Graduate of a school of professional nursing approved by the appropriate accrediting agency and accredited by ACEN or CCNE.
• Current, full, active, and unrestricted registration as a graduate professional nurse in a State, Territory, or Commonwealth of the United States, or the District of Columbia.
• Master’s degree in nursing (MSN) or Doctoral degree in Nursing with 2 years of specialized nursing experience, one of which is equivalent to Nurse II and meets all dimension requirements for Nurse III.
• Master’s degree in a related field with a BSN and 2 years of specialized nursing experience, one of which is equivalent to Nurse II and meets all dimension requirements for Nurse III.
• Doctoral degree in Nursing with 1 year of specialized nursing experience equivalent to Nurse II and meets all dimension requirements for Nurse III.
• Doctoral degree in a related field with a BSN and 1 year of specialized nursing experience equivalent to Nurse II and meets all dimension requirements for Nurse III.
