Nurse Care Manager & Patient Advocate (RN)
About the Role
Unicity Care Management supports individuals and families through critical healthcare decisions such as new diagnoses, complex hospital discharges, and chronic conditions that exceed what a family can coordinate alone. As a Nurse Care Manager and Patient Advocate you will be the first point of contact for clients and their families, providing high‑touch, concierge‑level nursing support in the community.

- Develop and maintain individualized care plans, revising them as circumstances change.
- Coordinate physicians, home‑care providers, specialists, and community resources into a single coherent plan.
- Manage transitions of care between hospital, rehabilitation, and home, closing gaps where clients are often lost.
- Advocate for clients and families at appointments, care conferences, and decision points.
- Serve as the primary point of contact for your caseload, responding to urgent needs outside standard business hours.
- Provide field‑based, high‑touch nursing support, including accompanying clients to medical appointments and other care‑related visits as needed.
Requirements
- Active Registered Nurse (RN) license in New Jersey.
- Genuine interest in working alongside families, not only patients.

- Valid driver’s license and reliable transportation for field visits.
Pay
Base salary $100,000 – $115,000 plus a monthly performance bonus earned on billable hours (paid each month, never deferred). First‑year total earnings typically range from $106,000 to $130,000+.
Schedule
Weekday hybrid schedule with genuine flexibility to build your own week. No hospital shift rotation, no twelve‑hour shifts, and no charge nurse or floor assignment. Field‑based role requiring regular travel; on‑call for urgent needs outside standard business hours.
