Join Our Team as a Nurse Care Manager with the Katie Beckett Program. Are you passionate about supporting children under the age of 18 with disabilities and/or complex medical needs as they grow and thrive in their homes and communities? Katie Beckett is seeking a dedicated Nurse Care Manager with pediatric experience to join our team. In this role, you will empower families caring for these children by providing services the member needs to attain financial, physical, and emotional relief. The position is with VSHP Volunteer State Health Plan, Inc.

Responsibilities

• Manage a caseload of multiple complex pediatric members
• Conduct member visits including home visits meeting timelines per contract
• Perform new member visits and ongoing assessments
• Develop and maintain Person‑Centered Support Plans
• Collaborate with private insurance (TPL) and Medicaid to coordinate care and ensure services are covered by the appropriate payor source
• Support utilization management functions for complex and non‑routine cases as needed
• Serve as a liaison between members, providers and internal/external customers in coordination of health care delivery and benefits programs
• Oversee highly complex cases identified through various mechanisms to ensure effective implementation of interventions and efficient utilization of benefits
• Perform essential case‑management activities: assessment, planning, implementation, coordination, monitoring, outcomes and evaluation
• Effectively communicate via digital channels and provide technical support
• Maintain an 18‑month commitment before pursuing other internal positions (effective 7/22/13)
• Complete required immunizations and/or associated medical tests
• Undergo digital literacy assessment
• Be flexible, adaptable, and willing to travel for face‑to‑face meetings with members
• Reside in one of the specified counties: Marshall, Bedford, Coffee, Maury, Mt Juliet, Murfreesboro, Rutherford, Sparta, Warren & Williamson
• Operate a BCBST‑owned or personal/rental vehicle for company business on a routine basis and comply with the BCBST Driver Safety Program (valid driver’s license, appropriate auto insurance, acceptable MVR)

Requirements

• Registered Nurse (RN) with active license in Tennessee or hold a license in the state of residence
