Nurse Care Manager
Position Overview
Navigates and coordinates the care of patients across the continuum of care. Monitors and manages resources to transition patients to the appropriate level of care in a cost-effective fashion while ensuring quality outcomes and patient satisfaction. Assesses, plans, coordinates, monitors and evaluates the patients’ plan of care in collaboration with the healthcare team. Monitors the patient care process and collects data to evaluate and improve care and collaborates with physicians and other health care workers to provide comprehensive care including health risk appraisal, health promotion and clinical outcome studies. Educates payers, providers and the public and serves as the information source regarding state and national health care trends; initiatives, which enhance the provision of, care and ensure efficient resource utilization.

Compensation
$40.70 - $61.05 per hour, depending on experience
Education/Experience
- Minimum Required: Associate’s in Applied Science in Nursing
Minimum of two (2) years of experience in acute/tertiary facility - Preferred: Bachelor’s of Science in Nursing
Five (5) years of experience in acute/tertiary facility
Licenses/Certifications
- Minimum Required: NYS Registered Nurse

Compensation Range: $40.70 - $61.05 per hour, depending on experience
