Nurse Care Coordinator

Patient Care

• Develop and implement individualized care plans in collaboration with patients, families, and healthcare teams.
• Care coordination and monitoring of patient care activities, ensuring continuity and quality of care.
• Serve as a primary point of contact for patients and families, facilitating communication and addressing concerns.
• Manage medication alerts, hospital admissions, and ER visits.
• Provide patient education on health conditions, treatment plans, medication management, and self-care strategies.
• Advocate for patients' rights and needs to ensure appropriate care and support.
• Conduct comprehensive assessments of patients' healthcare needs including medical, social, and behavioral aspects.
• Monitor patients' progress and adjust care plans as necessary to achieve optimal health outcomes.
• Coordinate referrals to specialists, community resources, and other healthcare providers.
• Provide specialized patient education as needed in an individual or group setting.
• Support one provider with direct nursing care and health education to patients.
• Maintain accurate and up-to-date patient records, ensuring confidentiality and compliance with data protection regulations.
• Document and manage patient care using the electronic health record.
• Administer vaccinations and manage chronic and acute health conditions.
• Collaborate with providers to ensure timely and effective delivery of immunizations and treatments.

Community Outreach and Health Programs

• Develop and implement health promotion and disease prevention programs tailored to the needs of the community served by the FQHC.
• Collaborate with community organizations and agencies to enhance health services and resources available to patients.
• Participate in community health fairs, screenings, and educational events.
• Manage and/or participate in health center programs such as Show Me Healthy Women, Vaccines for Children, Ryan White, Infection Control, STD disease monitoring, Primary Care Home Health, or specific disease monitoring and impact such as diabetes, hypertension, hepatitis C or HIV, etc.

Quality Assurance and Compliance

• Ensure compliance with healthcare regulations, standards, and best practices, particularly those specific to Federally Qualified Health Centers.
• Monitor and evaluate the quality of patient care, identifying areas for improvement and implementing corrective actions.
• Participate in the development and implementation of policies and procedures to enhance patient care quality and safety.
• Maintain patient confidentiality in accordance with HIPAA regulations.
• Adhere to health center safety, infection control, and hazardous waste procedures.
• Coordinate vaccine and medication management, ensuring proper storage, handling, and administration in accordance with established protocols and guidelines.
