Nurse

About the role

We are seeking Nurses who believe in our mission to help patients live out their best lives at home. This role involves coordinating care and ensuring quality care delivery within the Agency’s scope of services, policies and procedures, state and federal laws and regulations, the Nurse Practice Act, and Accreditation entity requirements.

This is a part-time position: Fridays only, 7:15 AM–5:15 PM.

Responsibilities

• Assess the continuing needs of the patient.
• Perform initial and ongoing patient assessments based on Agency policy and standards of practice to ensure effective and appropriate home care services.
• Participate with a team of RNs, LPNs, and/or Home Health Aides to provide quality home care services.
• Promote personal safety and a safe environment for patients and coworkers.
• Perform job duties in accordance with Agency policies, procedures, and professional and community standards.

Requirements

• Graduate of an accredited school of professional nursing.
• Home Care Experience preferred.
• Consistently demonstrates competency with technical nursing skills according to personal and legal scope of practice.
• Communicates effectively and in accordance with Agency requirements.
