About the role

As a Caring Hospice Registered Nurse Case Manager, you will perform ongoing patient assessments, maintaining thorough and diligent notes and records regarding the effectiveness of each patient's care plan. You will also keep in constant contact with patient family members and alternative caregivers.

Responsibilities

• Providing nursing care with an emphasis on pain and other symptom management
• Supervising Home Health Aides
• Referring patients' families to applicable community resources
• Maintaining strict patient confidentiality
• Providing patient care directives to family members and alternative caregivers

Requirements

• Licensed as a Registered Nurse in the state of Pennsylvania
• Associate/Bachelor's/Master's Degree in nursing
• Minimum of 2 years Med-Surg., Home Care, and/or Hospice experience
• Understand the difference between Progressively Declined and Custodial Care
• Ability to occasionally lift up to 25 lbs

Skills

We are seeking an RN who is energetic, organized, and emotionally capable of caring for terminally ill individuals. You must also be a team player, able to work harmoniously with a wide variety of individuals.

Schedule

• Shifts: 8:30 am – 5:00 pm
• Must be available to work occasional weekends
• Some holiday coverage required

$6,000 signing bonus
