About the role
Under general direction of the Chief of Internal Medicine and Family Medicine, this position assumes full responsibility for the rendering of professional medical services to patients in an ambulatory care setting. It works in a collaborative team practice that recognizes the patient as the center of the team.
Responsibilities
- Comprehensive, longitudinal clinical and administrative coordination of all care for a specific panel of patients.
- Examination, diagnosis, and treatment of patients in a primary care setting, including physical assessment and treatment.
- Supervision of care plan, including consultations, referrals, and communications with and to specialists as appropriate.
- Perform comprehensive health and developmental exams through baseline histories, review of systems, and physical exams.
- Differentiate normal from abnormal findings and adjust treatment regimens based on patient's response to treatment.
- Ensure coordination of on-going management of patients with certain chronic diseases between primary care team, subspecialty clinicians, and case management.
- Partner with patients and families to manage care plan, demonstrating empathy and effective communication skills in patient and family interactions.
- Consult and collaborate with colleagues in a team-based model of care.
- Prescribe medications in accordance with Massachusetts statutes and professional guidelines.
- Review incoming clinical data and follow up in a timely manner.
- Provide consultation regarding hospitalized patients in the panel.
- Efficiently and fully utilize the electronic medical record to manage practice through effective communications and documentation, including inbox management.
- Adhere to standards of daily task management as set by department, productivity and panel standards, and quality and patient experience performance measures.
Requirements
- Minimum Education: Graduate of a state-approved Program for Nurse Practitioner or Physician Assistant. Master’s degree required.
- Minimum Work Experience: 2 years of Primary Care experience, preferred.
- Licenses/Registrations: Nurse Practitioner - Board of Registration in Nursing (Massachusetts) or Physician Assistant - Board of Registration of Physician Assistants (Massachusetts).
- Required Classes/Skills: BLS - Basic Life Support Required.

Qualifications
- National certification as a Nurse Practitioner or Physician Assistant.
- Mass DPH Controlled Substances registration and DEA registration.

Pay
$107,000.00 - $224,123.00
Schedule
M-F, additional six weekends per year and one holiday every two years.
