About the role

The S. Nampa Clinic is part of St. Luke's Consumer Access & Experience (CAE) department. CAE collaborates with internal and external partners to innovate and improve clinical programs and processes.

Responsibilities

• Provide comprehensive patient care, including management of acute and chronic health issues.
• Assess patient cases, prioritize needs, and collaborate with specialists.
• Interface with specialists to facilitate shared decision-making.
• Document patient interactions in the EPIC EMR system.
• Perform post-discharge outreach to reduce readmissions.
• Collaborate with care team members to develop, refine, and enhance clinical programs and processes.

Requirements

A full-time (1.0 FTE) position as an Advanced Practice Provider (Physician Assistant or Nurse Practitioner) is available. A flexible mindset and interest in consumer insights are desired.

Qualifications

• Advanced Practice Provider license in good standing.
• Experience in internal or family medicine.
• Strong clinical skills and ability to prioritize patient needs.
• Proficiency in EPIC EMR system.

Skills

• Excellent communication and collaboration skills.
• Ability to work in a fast-paced, multidisciplinary team setting.
• Interest in consumer insights and design thinking.
