NP/PA

About the role

This is a full-time, on-site NP/PA role located in Monsey, NY. The clinician will provide direct patient care in a sub-acute setting, including conducting comprehensive assessments, developing and updating care plans, managing chronic and acute conditions, and monitoring patients' functional progress.

Responsibilities

• Collaborating with physicians, therapists, nursing staff, and facility leadership to coordinate rehabilitation and pain management, optimize clinical outcomes, and reduce avoidable hospital transfers.
• Documenting encounters thoroughly, ensuring regulatory and facility compliance, participating in interdisciplinary meetings, and communicating effectively with patients, families, and care teams.
• Reviewing medications to minimize polypharmacy, supporting restorative care goals, and contributing to quality improvement initiatives within partner facilities.

Qualifications

• Clinical expertise in physiatry, rehabilitation medicine, or sub-acute/long-term care, with strong skills in patient assessment, treatment planning, and pain management.
• Proficiency in primary care management, including chronic disease management, medication review, and reducing avoidable hospitalizations and narcotic dependence.
• Strong communication and collaboration skills for working with interdisciplinary teams, facility staff, patients, and families, with a commitment to patient-centered care.
• Solid documentation and EMR skills, with attention to detail and understanding of regulatory, quality, and compliance requirements in post-acute settings.
• Current licensure and certification as a Nurse Practitioner or Physician Assistant in New York State; board certification and relevant specialty experience preferred.
• Ability to work independently on-site, manage a dynamic caseload, prioritize tasks, and adapt to changing patient needs in skilled nursing facilities.
• Prior experience in skilled nursing, post-acute rehabilitation, or pain management strongly preferred; experience reducing polypharmacy and managing complex geriatric patients is beneficial.
