About the role

Lifestream Health Center is a leading provider of pain management and mental health services seeking a Nurse Practitioner/Physician Assistant to join their growing team.

Key Responsibilities

  • Perform thorough patient assessments, formulate accurate diagnoses, and develop individualized treatment plans.
  • Manage same-day patient visits and ensure all documentation is completed accurately and timely (within 72 hours).
  • Provide in-house treatments such as trigger point injections and pharmacy refills.
  • Facilitate appropriate referrals for imaging services, Physical Therapy, Chiropractic care, Orthopedic consultations, and laboratory services.
  • Work collaboratively with the team to ensure seamless care delivery, efficient clinic flow, and robust patient support.

What You Bring

  • A current and unrestricted Nurse Practitioner/Physician Assistant license in Maryland.
  • An active CDS and DEA licenses.
  • Experience in pain management, mental health, behavioral health, or addiction medicine is preferred.
  • Demonstrated strong clinical judgment, excellent time management, and meticulous follow-through.
  • A patient-first mindset, committed to providing compassionate and comprehensive care.
  • Comfort and proficiency working independently in a satellite clinic setting, with full remote support.
