Position Overview
As an LPN, you will provide direct nursing care to residents while working collaboratively with RNs, physicians, and the interdisciplinary care team. This role is essential to supporting residents’ recovery, comfort, and overall well-being in our long-term care and rehabilitation setting.
Key Responsibilities
- Administer medications and treatments as prescribed by physicians
- Monitor, document, and report changes in residents’ health status, including vital signs
- Absorb, assist residents with activities of daily living (ADLs) such as bathing, dressing, and feeding as needed
- Collaborate with the rehabilitation team to support recovery goals, including physical, occupational, and speech therapy plans
- Maintain accurate and timely documentation in accordance with facility policies and state and federal regulations
- Provide emotional support and education to residents and their families
- Ensure a safe, clean environment by following infection control and safety protocols
Qualifications
- Current and valid Licensed Practical Nurse (LPN) license
- Strong clinical skills, including medication administration and wound care
- Excellent communication and interpersonal skills
- Ability to work effectively in a team-oriented environment
- Career-focused approach to care
- BLS/CPR certification required
