- Fellowship Trained in Neuromuscular or Neurophysiology
- Board Eligible or Board Certified in Neurology
- PA Medical License
- PhD/MD Graduate of accredited medical school with degree of Doctor of Medicine (MD) or Doctor of Osteopathy (DO)
