Neurology Medication Therapy Management Pharmacist

Responsibilities

• Provide comprehensive medication reviews, medication reconciliation, and patient education to optimize medication therapy outcomes.
• Collaborate with providers to adjust therapies under Collaborative Practice Agreements and recommend evidence-based treatment plans.
• Order and monitor appropriate laboratory tests to support safe and effective medication use.
• Assist patients with medication affordability and adherence strategies.
• Lead and participate in quality improvement initiatives, workflow enhancements, and standards-of-care development.
• Educate clinic staff and interdisciplinary teams on medication-related best practices.
• Maintain compliance with regulatory requirements while demonstrating a commitment to exceptional patient-centered care.

Qualifications

• Education: Doctor of Pharmacy (PharmD) required.
• Board Certification in Ambulatory Care, Geriatrics, or a related specialty preferred.
• Experience: 3–5 years of pharmacist practice experience, or completion of a PGY1/PGY2 pharmacy residency.
• Licensure & Certifications: Current pharmacist license in the state where MTM services are provided. Ability to obtain and maintain pharmacist licensure in additional UnityPoint Health service area states as needed.
