- Board Eligible/Certified in Neurology (ABPN)
- Fellowship-trained in Headache Medicine
- UCNS-certified
- Additional certification in Headache Medicine preferred
- MD/DO from an accredited institution
- Active/unrestricted license or eligibility
- Completion of credentialing
