We are seeking an experienced Medical Director with a background in Cardiology, Nephrology, or Primary Care to lead an integrated clinical care model focused on primary care coordination and specialty collaboration. The role involves providing clinical and administrative leadership to a multidisciplinary team, including nurse practitioners, physician assistants, and nurse case managers, to enhance patient outcomes by reducing hospitalizations, emergency visits, and readmissions.
