NCM RN Nurse Care Manager

You'll collaborate with interdisciplinary teams to identify and address patients' needs, and provide education and support to patients and their loved ones. Responsibilities

• Coordinate care plans with patients, families, and healthcare teams to ensure seamless transitions and optimal outcomes
• Conduct initial and ongoing assessments to identify patient needs and develop individualized care plans
• Collaborate with interdisciplinary teams to address patient concerns and develop solutions
• Monitor patient progress and adjust care plans as needed to ensure optimal health outcomes
• Maintain accurate and timely documentation of patient records

Requirements

• Current NY State RN license
• Strong communication and interpersonal skills
• Ability to work in a fast-paced environment with multiple priorities
