Job Summary

At Easterseals PORT Health (ESPH), our mission is to empower individuals and strengthen communities by providing support to those living with mental health challenges and disabilities. We are seeking a compassionate and knowledgeable NC START Clinical Coordinator to join our multidisciplinary team.

About the Role

Your role involves implementing the START model, collaborating with cross-system teams, conducting clinical assessments, providing psychoeducation, and supporting individuals with intellectual and developmental disabilities (I/DD) and significant behavioral needs in community-based settings.

Responsibilities

  • Implementing the START model for crisis prevention and intervention
  • Collaborating with cross-system teams to develop and refine crisis prevention plans
  • Conducting formal and informal clinical assessments to guide care recommendations
  • Providing psychoeducation and consultation to families, providers, and community stakeholders
  • Serving as a liaison between individuals and community resources
  • Rotating on-call coverage, including weekends

Requirements

  • Master's degree from an accredited university in Social Work, Counseling, Psychology, or a closely related field
  • Licensed in your field (license preferred)
  • Qualified Professional (QP) status
  • Experience with individuals with intellectual/developmental disabilities, mental health diagnoses, or both, particularly those with significant behavioral needs
  • One year of working with children and adolescents
  • Willingness to travel within the central NC region for assigned cases and on-call responsibilities
  • Valid driver’s license, insurance, and reliable transportation

  • Top-tier verbal and written communication skills
  • Ability to provide services in various community locations within the region as needed
  • Willingness to rotate crisis line coverage for 1-3 days at a time
