Navigator RN - Hematology Oncology

Role Overview

The Nurse Navigator in Infusion is a clinical expert responsible for coordinating and delivering personalized patient care for individuals diagnosed with cancer. They assist patients and families in navigating hospital and community services, providing education, health screenings, and appropriate referrals. The role involves managing treatment regimens, collecting data on contacts and referrals, and building relationships with patients, families, healthcare providers, and community partners. The Nurse Navigator advocates for whole-person health, integrating physical, mental, and spiritual wellness, while demonstrating core values aligned with the organization's mission.

Key Responsibilities

• Coordinate and administer treatment plans and patient care
• Provide patient education, health screenings, and referrals
• Cultivate relationships with patients, families, providers, and community entities
• Manage data related to contacts, referrals, and treatment administration
• Advocate for holistic, personalized care aligned with organizational values

Qualifications & Skills

• Bachelor's Degree in Nursing
• California Registered Nurse License
• Oncology certification (e.g., OCN, AOCN, CBCN) or successful completion within 1 year
• At least 5 years of nursing experience, including 3+ years in oncology
• Current chemotherapy administration certification
• Preferred: Master's degree in Nursing
