About the role
The Navigator II's primary role is to enhance the likelihood of success, identify and reduce barriers for clients and connect them with needed services and resources. The Navigator II will work with individuals who have been determined as in need of additional support and engagement and will be responsible for conducting outreach, engagement, case management, and ongoing wrap around supportive services. The Navigator II will also map out the most appropriate next steps for the individual who may need assistance in accessing various services to achieve ongoing success. The Navigator II is part of a multi-disciplinary team made up of internal and external stakeholders.
Responsibilities
- Conduct outreach, engagement, case management, and ongoing wrap around supportive services for individuals in need of additional support
- Map out appropriate next steps for individuals needing assistance accessing various services to achieve ongoing success
- Document and track required patient interactions according to Arizona Administrative Code (AAC), ADHS/DBHS Provider Manual, CARF, and Community Bridges Policies and Procedures
Requirements
- High school diploma or GED required
- Associate degree (or higher) in a field related to Behavioral Health preferred but not required
- Minimum of 12 months of recovery from substance use and/or mental health disorders required
- 1-3 years of full-time Health Care related work experience providing adequate exposure to both behavioral and medical crisis situations preferred but not required
- 1-3 years of substance abuse/mental health recovery-oriented service preferred but not required
- Minimum of ninety (90) days experience in a Navigator or Peer Support Specialist role required
- Current AZ Driver's License (valid and in good standing)
- Must be able to provide a 39-month MVR
- Arizona Fingerprint Clearance card (program specific)
