MVM-MDS Coordinator
About the role
The MDS Coordinator is responsible for coordinating and completing all required Resident Assessments Instruments and ensuring accuracy of information. Notify the Interdisciplinary Team (IDT) of any barriers to the timely completion of all residents' Assessment Instruments. Coordinate and facilitate Resident Care Plan Conferences on a quarterly basis for all residents, ensuring that the resident and the responsible party receive a written notification of the meeting specifying date, time, and place.
Responsibilities
- Review of each Medicare resident's case on the day to admission to ensure appropriateness of Medicare coverage.
- Active interaction with IDT on a weekly basis to evaluate resident's status and needs relative to Medicare skilled needs and potential for RUG category changes.
- Conducts review via chart audit to ensure the residents receive the care and services they need in a timely manner, initiate change of condition RALS when appropriate, and ensure chart documentation, including nursing and ancillary notes, care plans reflect the need for care and services the resident receives.
- Discharge planning proceeds smoothly relative to resident's progress and Medicare coverage status. Medicare coverage is discontinued when resident no longer meets the criteria.
- Reports routinely to the Director of Nursing regarding Medicare status.
- Conducts quarterly audits to include: MDS, Care Plans, Psychotropic and any other as needed. Ensures that the Physician Certification/Re-Certification for Medicare A residents are completed on time.
- Subject to additional duties such as working on the floor if needed. Other duties as assigned by the DON or facility administration in accordance to facility needs, including participation in Call Schedule.
Requirements
- RN currently registered in the state of Montana.
- Previous work experience in long-term care setting and is listed in a good standing with the Montana State of Professional licensing.
- Freedom from illegal use of drugs and freedom from use and effects of drugs and alcohol in the workplace.
- Pass a background check.
Qualifications
- Working knowledge and ability to apply professional standards of MOS Coordinator practice in job situations.
- Establish criteria to assure that the service provided meets established standards of quality.
- Current knowledge of state and federal laws and regulations that apply to practice of MOS Coordinator in long-term care.
- Interpersonal skill to interact with the resident, families, community and government agencies, and interdisciplinary staff in a non-judgmental, supportive and calm manner.
- Management skills sufficient to allocate resources and to supervise interdisciplinary cooperation in the planning and implementation of individual resident care plans.
- Able to apply facility policies of facts leading to job actions without regard to race, religion, age, national origin, sex, or disability.
- Able to express self adequately in written and/or oral communication and to communicate effectively in an interdisciplinary care setting the residents, families, staff members, representative of community and government agencies.
- Carries out all duties in accord the facility mission and philosophy.
- Demonstrates knowledge of and respect for the rights, dignity and individuality of each resident in all interactions.
- Able to serve as role model for staff.
- Able to appreciate the importance of maintaining confidentiality of residents and facility information.

- Knowledge of emergency and disaster procedures of facility, able to locate nearest exit, to understand and respond to written or oral instruction in care of an emergency.
- Sufficient mobility, and strength to move freely through the building, to assure resident safety at all times and to assist, transfer or otherwise move residents of the facility out of danger in case of an emergency.
- Able to arrive and to begin work on time and report for duty as scheduled on all shifts, weekends and holidays.
- Working knowledge and ability to comply with facility policies and procedures for workplace safety including infection control procedures, application of universal precautions for blood borne pathogens, use of personal protective equipment and handling of hazardous materials.
- Able to prioritize tasks/responsibilities and compete duties/ projects within allotted time.
- Able to respond to change productively and to handle additional tasks/projects as assigned.
- Able to respond to facility and resident needs on an on-call basis, this position shares On-Call duties.
- Able to carry out the essential functions of this job (with or without reasonable accommodation) without posing specific, current risk of substantial harm to health and safety of self and others.

Pay
Salary range: $50,000 - $70,000 annually, commensurate with experience.
Schedule
Full-time position, subject to call-offs.
