MUSCP - Care Coordinator (RN) - Department of Medicine, Division of Pulmonary

MUSC Division of Pulmonary, Critical Care, Allergy, and Sleep Medicine is seeking a nurse coordinator to help lead this dynamic team into the future. We are looking for an individual who is motivated to build relationships with both team members and patients to enhance clinic efficiency, education, community outreach, and research to improve patient outcomes.

About the role

The General Pulmonary nurse coordinator will participate as an expert practitioner in a variety of direct and indirect patient care activities that will enhance the overall quality of clinical care of the general pulmonary population. This position reports directly to the Division Administrator with a matrix reporting structure involving all applicable divisional faculty.

Responsibilities

• 35% - Coordination of Clinic:
  • Coordinate new patient referrals in collaboration with central and pulmonary division scheduling to ensure efficient access with the appropriate provider.
  • Review new patient visit planning and scheduling of testing to ensure correct information and appropriate providers are scheduled.
  • Oversee clinic standardization of orders for new referrals and return patients in collaboration with current resources.
  • Manage medication including prescription refills, monitoring for drug toxicity, and prior authorizations.
  • Lead quality improvement projects, determined in collaboration with the supervising physician, to ensure standardized and efficient care.
• 30% - Patient care:
  • Serve as the primary coordinator of care for general pulmonary patients.
  • Participate in the evaluation and treatment planning of patients in the outpatient clinic, including new referrals, routine follow-up visits, hospital follow-up visits, and acutely ill patients.
  • Coordinate outpatient consultations, therapies, and treatment plans.
  • Advise nursing staff on patient management when contacted by patients for medical problems.
  • Identify and provide resources to support patient needs, including community resources and access to basic needs.
  • Serve as the voice of the patient across care settings and empower patients to problem-solve care options while promoting their right to self-determination.
• 30% - Education:
  • Develop education resources for patients in collaboration with supervising physicians to empower patients and caregivers for improved quality of life and adherence to complex medical regimens.
  • Create materials to highlight the clinical research portfolio for patients, caregivers, and referring physicians.
  • Serve as a resource to identify potential patients eligible for clinical trials and collaborate with MUSC research staff.
  • Provide guidance to clinic nursing and CMA staff on common adverse events in the patient population and ensure a standardized approach to management in collaboration with supervising physicians.
• 5% - Communication and Outreach:
  • Collaborate with MUSC marketing to enhance outreach for the program across the state and region.

Requirements

• Bachelor’s degree in nursing is required.
• Three (3) years of related nursing experience.
• Licensure as a Registered Nurse by the South Carolina Board of Nursing or a compact state.
• Some positions require certification as a generalist in a related specialty area by the American Nurses Association (ANA).
• Current American Heart Association (AHA) Basic Life Support (BLS) certification or American Red Cross BLS for Healthcare Providers certification is required.

Physical Requirements

• Continuous standing, walking, and sitting; ability to bend at the waist, kneel, climb stairs, and reach in all directions.
• Fully use both hands and legs, possess good finger dexterity, and perform repetitive motions with hands, wrists, elbows, and shoulders.
• Qualified physically for respirator use, initially and as required.
• Maintain 20/40 vision corrected, see and recognize objects close at hand and at a distance.
• Work in a latex-safe environment and work indoors.
• Frequently lift, lower, push, pull, and/or carry objects weighing up to 50 lbs unassisted; exert up to 50 lbs of force; lift from 36” to overhead up to 25 lbs.
• Infrequently work in dusty areas and confined/cramped spaces.

Pay

This is a salaried position at pay grade Health-29.
