Our Social Workers provide emotional support, counseling, and resource coordination to patients facing terminal illness. This role plays a key part in the delivery of holistic care and ensures that patients' psychosocial needs are met with dignity and respect. Schedule

Mon-Fri, 8:00 AM - 5:00 PM

Responsibilities

• Assess psychosocial needs of patients and families
• Evaluate home safety; assist with alternative care plans when needed
• Provide counseling, crisis intervention, and emotional support
• Maintain accurate and timely clinical documentation
• Educate and assist with advance directives
• Provide referrals and practical support for patients and families
• Serve as liaison with community resources and agencies
• Collaborate with team members to support holistic patient care
• Support funeral
