POSITION SUMMARY
Conducts psychosocial assessments, coordinates discharge planning and community resources, provides counseling and patient/family support, and collaborates with the interdisciplinary team to promote patients' functional recovery and safe transition to the next level of care.

Minimum Required Qualifications
- MSW
- BLS
- +1 year social work experience in an acute hospital setting
