MSW- Medical Social Worker- Home Health

About the role

Visiting Nurse Health System (Visiting Nurse) is seeking a Medical Social Worker with experience in the Home Health space. The ideal candidate provides social, emotional, and psychological services/counseling to terminally ill patients and their bereaved families, enabling the patient to maintain independence and remain at home. The Home Health Medical Social Worker will facilitate alternative care placement requested by patient/family to an appropriate facility.

Responsibilities

• Demonstrates the skill and knowledge in counseling patients/families using a variety of clinical theories of short- & long-term interventions; including identifying social, emotional, financial & environmental needs/issues and develop appropriate intervention plan
• Demonstrates sensitivity and skill in discussing loss issues with both children and adults
• Demonstrates skill to assess the patient /family’s coping ability e.g. strengths, weakness, and socio-cultural environment to develop a sound base for an instructional plan
• Demonstrates knowledge of agency policies and procedures related to social work to provide services within approved social worker parameters
• Demonstrates knowledge of eligibility criteria for federal and state services such as Medicare, Medicaid, AHS, SSI, Welfare, Veteran’s benefits, etc.
• Affords assistance to patients/families to receive benefits to which they are eligible
• Completes initial assessment on patient and family within seven days of admission to the hospice home care program or residential center
• Affords assistance to patient/patient family with necessary forms, applications, documents in a timely manner that are required by outside agencies (i.e., OLPH, SWC applications, Medicare/Medicaid revocation/election/re-certification forms)
• Ensures that documentation is consistent with the format required by departmental standards (i.e., progress notes reflect care plans)
• Displays the ability to follow through in a thorough and timely manner on tasks assigned by management team and requests made by patients/families, referral sources, and community

Requirements

• Master’s degree in social work from an accredited institution of higher learning
• GA licensure; LCSW preferred
• Must have reliable transportation, valid driver's license, and current vehicle insurance
• Minimum of 1 year experience

Pay

Range Salary Range is from $56,000- 84,000
