About the Role
Responsible for performing all aspects of the provision of patient social services. Establishes, implements, and evaluates goals and objectives for social services to meet and promote the standards of quality and contribute to the organization's mission and philosophy. Service coverage area: North San Diego County.
Responsibilities
- Conducts Explanation of Benefits
- Directs all daily patient social service operations including providing direct oversight of the establishment and implementation of social service policies
- Assures proper documentation of social services in clinical records
- Maintains comprehensive working knowledge of the field of social work
- Maintains comprehensive working knowledge of community resources and assists referral sources in accessing community resources

Requirements
- Master's degree in Social Work
- At least three (3) years experience preferably in hospice
- Ability to deal compassionately and kindly with patients and the community
- Bilingual highly desired
