About the Role
Make a meaningful impact supporting some of New York City's most vulnerable adults. We're seeking a compassionate and organized Social Worker to provide case management, advocacy, referrals, and community-based support to clients throughout the five boroughs.
Responsibilities
- Manage and monitor an assigned client caseload
- Assess client needs, safety, functioning, and eligibility for services
- Advocate for clients and develop individualized service plans
- Coordinate referrals for Medicaid, Medicare, SNAP, home health aides, public assistance, legal aid, medical/psychiatric evaluations, protective services, and more
- Conduct home, facility, and community visits throughout NYC
- Attend court hearings and community meetings
- Maintain case records and documentation using HRA and agency databases
- Collaborate with community agencies, healthcare providers, legal teams, and other professionals
Qualifications
- Master's degree in Social Work, Counseling, Psychology, Gerontology, or a related field
- Strong writing, documentation, organizational, and time-management skills
- Computer proficiency
- Ability to travel throughout all 5 boroughs
- No license required
Pay
$25–$29/hour DOE
Schedule
Full-Time | Monday–Friday | 9:00 AM–5:00 PM. Certain emergency situations may occasionally require work outside the normal workweek, including evenings, weekends, or holidays.
