Essential Job Functions
- Conducts Michelle P. Waiver assessments and reassessments, including a face-to-face visit within the 14-day deadline.
- Completes initial assessments for new individuals to the Michelle P. Waiver program.
- Provides information on service types and directs individuals to available case management agencies.
- Documents responses using a fifteen-page assessment tool covering health, ADLs, neurological and emotional status, social supports, and physical environment.
- Obtains consents and completes assigned forms, ensuring documents are securely shared with the individual and medical records staff.
- Enters information notes and assigned documents into the electronic medical records system within given deadlines.
- Submits MPW evaluations to the Quality Improvement Organization for Level-of-Care approval or denial, and assessment/reassessment to the MPW provider upon approval.
- Communicates with individuals, their families, and case managers on the status of their assessment throughout the evaluation process.
Education and Experience
- Registered nurse or social worker with an active and unencumbered RN license, or Bachelor’s degree in social work, sociology, psychology, or a related field as approved by Medicaid.
- One year experience with the DD/ID population required, or six months’ experience with the population and a Master’s degree.
- Working knowledge of the Michelle P. Waiver and eligibility criteria.
Physical Demands
- No unusual physical demands.
- Requires travel to offsite locations and may involve walking, standing, stooping, or bending.
- Extensive work in client’s homes, occasionally involving modestly unpleasant situations.
- May require occasional extended periods of sitting or standing.
