Description
The Mobile Team Nurse will oversee and monitor the health care needs of residents in the MRST East catchment area. Duties include crisis intervention, monitoring medical conditions, medication compliance, and providing education and support services.
Duties And Responsibilities
- Overall assessment of client welfare and appropriate intervention and follow-up as needed.
- Crisis intervention (including 24/7-emergency coverage).
- Making sure medical conditions are being properly managed and following medical provider instructions.
- Helping residents manage their medication regimens and providing assistance if needed.
- Providing education and resources about medications and medical conditions.
- Offering individual counseling and support services related to physical wellbeing.
- Timely submission of progress notes, assessments, and other relevant documentation.
- Aiding clients in making medical appointments and ensuring clear communication between client and provider.
- Collaborating with medication prescribers, medical service providers, case managers, etc., to ensure ongoing care.
- Keeping up-to-date with all required compliance documents and participating in necessary trainings.
Requirements
- Current NYS License and registration as a Licensed Practical Nurse.
- Minimum of two years experience working with people with mental health disabilities.
- Good verbal and written communication skills.
- Ability to make sound judgments regarding client care.
