Responsibilities
- Provides professional physical therapy services to home health clients
- Develops and implements individualized treatment plans
- Delivers preventive, therapeutic, and rehabilitative interventions
- Coordinates care with physicians and the interdisciplinary team
- Communicates changes in patient condition
- Ensures accurate and timely clinical documentation
- Performs patient and caregiver education
- Conducts environmental assessments
- Participates in discharge planning
- Engages in quality improvement activities
- Supervises PTA and Home Health Aides as permitted by state practice acts
Requirements
- Graduate of an APTA-approved Physical Therapy program
- Licensed Physical Therapist in the state of practice
- Minimum one (1) year of experience or completion of a 6–8 week preceptorship
- Home health experience preferred
- Ability to work independently with minimal supervision
- Commitment to maintaining clinical competence, organizational policies, and ethical standards
- Valid driver’s license and reliable transportation required
- Meets physical demands of the role, including lifting up to 35 lbs and travel to patient homes
