Responsibilities

• Face-to-face weekly sessions with the client
• Conduct home visits
• Aid in medication counts
• Aid in life skills support (resume building, scheduling, budgeting, organizing, goal setting and tracking, job support, personal accountability and follow through, etc.)
• Engage in recovery-related activities with their client (exercise, meditation, grocery shopping, 12-step meetings, community development, etc.)
• Provide phone support for client
• Maintain ongoing communication with case manager
• Document all sessions, interactions and anything noteworthy in a timely manner
