About the role
The Registered Nurse works as part of a Forensic Assertive Community Team (FACT) that provides care and treatment for clients experiencing significant mental health/psychiatric symptoms, functional impairments, and possible co-occurring chemical dependency and/or personality disorder. FACT is an adaptation of the assertive community treatment model designed to serve people with serious mental illness who are involved in the criminal justice system.
Responsibilities
- Work in full partnership with the client in their recovery process.
- Promote the care, welfare, treatment, and safety of clients with significant mental health/psychiatric symptoms and possible co-occurring conditions.
- Conduct health/medication assessments, crisis assessment and management, treatment planning, individual counseling, skills teaching, psychiatric/medical case management, medication monitoring/administration, staff training, and care coordination with outside providers.
- Provide crisis and behavioral management techniques, goal-directed counseling, and medical/psychiatric case management.
- Collaborate with mental health professionals and criminal justice agencies.
- Deliver nursing care, monitor treatment, and administer medication to individuals with mental health disorders and/or intellectual and developmental disabilities, following established nursing practices and guidelines.
- Coordinate communications and actions of various disciplines related to health and safety support for the client.
- Complete required nursing documentation (daily, monthly, quarterly, and annual) and ensure health records are accurately maintained.
- Provide on-call support as needed.
Requirements
- Associate degree in nursing with at least 6,000 hours of supervised experience in delivering services to persons with mental illness; or
- Bachelor’s degree in nursing with at least 2,000 hours of supervised experience in delivering services to persons with mental illness; or
- Graduate degree in nursing with at least 2,000 post-BA hours of supervised experience in delivering services to persons with mental illness.
- Ability to problem-solve, communicate effectively (orally and in writing), and manage relationships.
- Ability to work independently.
- Travel as needed.
Skills
- Certification as a Certified Psychiatric Rehabilitation Practitioner (supported by employer).
Physical Requirements
Medium Work: Exerting up to 50 pounds of force occasionally, and/or up to 30 pounds of force frequently, and/or up to 10 pounds of force constantly to move objects.
Pay
$90,000–$92,000 per year.
