About the role

As our Mental Health Professional, you will serve as a member of a multi-disciplinary team, assessing and providing support and intervention for the social and emotional needs of patients and their families to promote continuity of care and services. You will conduct psycho-social assessments and mental health evaluations, provide referrals and crisis intervention, offer environmental assistance and patient advocacy, support discharge planning, and coordinate legal needs for patients detained on Single Bed Certification, with assignments potentially spanning multiple locations.

Responsibilities

  • Perform assessments, evaluation, mental health screening and/or mental health assessment and evaluation, and referrals, focusing on the psychosocial needs of the patient and family.
  • Facilitate family conferences as appropriate.
  • Identify and communicate social, emotional and patient/family stressors to members of the care team as it relates to patient’s problems and plan of care.
  • Identify and facilitate safe discharge planning along the continuum; communicate and document plan to interdisciplinary patient care team.
  • Coordinate and complete legal needs activities for patients detained on Single Bed Certification, including but not limited to: filing of court petitions, completion of medication rights, communication and planning assistance with attorneys, court and the healthcare team.
  • Promote safety, quality and appropriate resource utilization; advocate on behalf of patients and caregivers for service access and for safety and rights (e.g. victims of domestic violence, sexual assault, disability access or community-based services).
  • Ensure compliance with state reporting requirements and appropriate documentation as identified by department policy and regulatory requirements/agencies; provide consultation to staff and physicians regarding reporting requirements.
  • Provide consultation and guidance to staff and physicians about patient psychosocial issues and needs.

Requirements

  • Licensed Mental Health Counselor: WA, within 24 months
  • Licensed Independent Clinical Social Worker: WA, within 24 months
  • Licensed Advanced Social Worker: WA, within 24 months
  • Licensed Marriage and Family Therapist/Counselor: WA, within 24 months
  • Agency Affiliated Counselor: WA, within 24 months
  • Social Worker Associate Ind Clin: WA, within 24 months
  • Licensed Independent Social Worker: WA, within 24 months

Preferred

  • Master’s degree in a behavioral health field (e.g. social work, psychology, counseling, etc.) upon hire
  • Two years recent related work experience in a healthcare setting
