Requirements

  • Masters degree in human services/related field
  • Year clinical experience working with adults with severe mental illness
  • Knowledge and understanding of recovery principles and recovery driven service system
  • Driver’s License and willingness to utilize personal vehicle for work-related tasks
  • Necessary to offer support across Tidewater Region
  • Licensed as an LPC or LCSW in Virginia preferred

Responsibilities

  • Assess, plan, and develop individualized treatment plans for individuals with severe and persistent mental illness
  • Cookout and provide therapy, medication management, and resource linkage services
  • Collaborate with a multidisciplinary team including psychiatrists, nurses, substance abuse professionals, employment specialists, housing specialists, and peer specialists
  • Conduct at least three home visits per week to deliver services in the individual's home or community setting
  • Utilize personal vehicles for client transportation and community-based work

Qualifications

  • Assertive Community Treatment (ACT) Team member
  • Deliver comprehensive services to individuals with severe and persistent mental illness
  • Support individuals with serious mental illnesses who may not have experienced success or haven't found effective support within traditional outpatient programs
  • Offer tailored services emphasizing relationship-building and active involvement in enhancing functioning and symptom management
  • Implement weekly home visits, provided either in individuals' residences or within the community, utilizing personal vehicles for client transportation and community-based tasks
