Mental Health Prof I Bach
Job Summary
Your goal as a mental health professional is to create discharge plans that address my patients' psychological, social, cultural, and financial needs. This helps them recover and maintain their overall well-being.
Responsibilities
- Conducts individual, group, and family psychoeducational groups.
- Contributes to treatment plan development and attends treatment planning meetings and reviews on an ongoing basis.
- Affirms environment and patient’s safety status and reports/responds to actual and potential safety issues or changes in the patient's condition.
- Identifies risk factors, screening criteria, and interventions for patients with special precautions (e.g., suicide, homicide, falls, elopement).
- Completes relevant assessments, group notes and all other documentation promptly and in compliance with requirements, as appropriate to scope.
- Collaborates with the multidisciplinary team to assess and coordinate patients’ discharge planning needs and readiness for discharge.
- Conducts post-discharge follow-up as indicated.
Qualifications
- Basic Cardiac Life Support must be obtained within 7 days of employment start date
- Bachelor's Degree (LMSW/RCSW-I), Lic Master Social Worker/Reg Clin SW-Intern, or (LPC/LMHC) Licensed Professional/Mental Health Counselor, or (LBSW) Licensed Baccalaureate Social Worker, or (LMFT) Licensed Marriage and Family Therapist
- No Travel Required
- 2 years experience Required
