About the role

Provide mental health and care coordination services to geriatric patients transitioning from hospital care to various living environments.

Responsibilities

• Conduct medication reviews and psychiatric assessments to evaluate patient needs.
• Develop, implement, and monitor individualized care plans tailored to each patient.
• Coordinate with patients, families, and multidisciplinary care teams to ensure seamless transitions and comprehensive support.
• Document patient progress and care activities accurately in facility and office settings.
