Mental Health Group Facilitator

About the role

A Group Facilitator is needed to lead TBS groups in nursing homes. A QMHS III or LSW certification is required.

Responsibilities

• Facilitate TBS groups focusing on emotional health, coping skills, and quality of life
• Adapt group content to meet the needs of older adults, including those with medical complexities or mild cognitive impairment
• Collaborate with facility staff and clinical team to provide consistent, compassionate care
• Document group sessions accurately and on time
• Encourage resident engagement and foster a safe, respectful group environment
• Participate in supervision and professional development

Qualifications

• Must hold QMHS III, LSW, LPC, or equivalent Ohio-recognized behavioral health credential
• Experience facilitating behavioral health or substance-use groups
• Experience working with older adults, long-term care, or medically complex populations preferred
• Strong communication skills, empathy, and patience
• Able to work independently in off-site facilities and travel as needed
• Must meet Ohio Medicaid and background check requirements

Preferred Skills & Attributes

• Warm, compassionate approach with a genuine interest in the wellbeing of older adults
• Experience with EHR systems (training available)
• Trauma-Informed Care training or willingness to obtain within 90 days
• Comfortable in nursing home environments and able to walk/stand as needed
• Flexible, adaptable, and comfortable traveling between locations
