Program Summary
Home & Community Based Support Services consists of three programs aimed at keeping children in their homes with their families by offering counseling, psychoeducation, skill building, support, and linkage.
Responsibilities
- Provide mental health assessment, diagnosis, treatment planning, and psychotherapy services
- Offer individual, family, and group counseling to a diverse population facing various challenges
- Implement interventions to develop and enhance social, interpersonal, self-care, and independent living skills for stability and functional gains
- Support families during and after crisis episodes involving children or families, aiming to stabilize the child/youth in the home and natural environment
- Maintain the crisis phone number, on call for a week at a time, rotating among staff
- Collaborate with other treatment providers and collateral contacts as necessary
- Complete and submit required documentation in a timely manner, such as progress notes, treatment plans, etc.
- Maintain positive working relationships with other professionals, community agencies, and team members
- Demonstrate sensitivity to cultural and ethnic norms
- Follow agency’s Worker Safety protocols
- Participate in team meetings, coaching/supervision meetings, and the agency Quality Improvement Process
- Engage and guide the youth and family through active listening skills and therapeutic engagement methods
- Maintain positive working relationships with program/agency staff, collaborating agencies, and stakeholders
- Continue professional growth and training in best practices, such as cultural competencies
- Attend coaching/supervision meetings as required
- Participate in the agency Quality Improvement Process and other agency meetings including in-service trainings, supervision, and staff meetings as requested
- Possess a working knowledge of Microsoft based products, including agency programs such as Anasazi and Fidelity HER
Requirements
- NYS Education Department license in Social Work, Mental Health Counseling, Psychology, or Marriage and Family Therapy
- At least one (1) year of experience providing direct services for children with one or more of the following primary diagnoses: mental illness, alcoholism, chemical dependency, and substance abuse
- Demonstrated cultural competence in understanding and working with disadvantaged/minority populations in community-based programs in an urban and suburban setting
- A valid NYS driver’s license is required
- Comfortable transporting clients in personal vehicle as needed
- Computer literacy required, experience with Anasazi preferred
- Excellent written and oral communication skills
