About the Role

The MST Coach is a specialized Case Planner role focused on the MST-FIT (Multi-systemic Therapy/Family Integrated Transition) model. Working within the social service department, the Coach supports youth and families by providing wraparound services to ensure successful discharge. This role involves case management using the MST approach, fieldwork, and occasional evening hours for parent conferences and home visits.

Our main campus in Rockaway Park, Queens, houses a Residential Treatment Center, Close to Home Non-Secure Placement programs, administrative offices, clinical services, and an on-site NYC Department of Education school. We also operate a Non-Secure Detention residence in Richmond Hill and Limited Secure Placement programs in Brooklyn and Staten Island.

Responsibilities

• Demonstrates fidelity to the agency’s service values and organizational imperatives.
• Provides aftercare support to cases transitioning to trial discharge using the MST-FIT model of treatment/therapy.
• Provides additional support to cases transitioning from trial discharge MST-FIT to return to placement as necessary.
• Provides crisis intervention and conflict resolution to youths and their families, including evenings/weekends as required on a consistent rotation schedule.
• Conducts virtual and in-person sessions with resident youth and families.
• Responsible for weekly documentation, including case, collateral, and other contacts within 48 hours of occurrence.
• Ensures ongoing support and collaboration across MST and ITM program staff on behalf of residents.
• Prepares and submits MST-FIT engagement court reports for scheduled permanency hearings.
• Prepares for and attends weekly and other ITM learning community meetings and trainings.
• Attends monthly mandatory trainings, including 7-day ITM and 5-day MST-FIT booster trainings.
• Prepares for and attends weekly supervision and MST-FIT group consultations as scheduled.
• Prepares for and attends monthly MST-FIT psychiatric consultations as scheduled.
• Supports the social service department with resident transitions using the MST-FIT model of treatment engagement.
• Performs fieldwork, which may include driving residents in your vehicle.

Requirements

• Minimum MSW degree; licensure preferred.
• Minimum of 1 year of experience working in child welfare with at-risk youth.
• Former case management experience is a plus.
• Valid NYS Driver’s License.
• Professional, mature, and dependable with a bias for action.
• Ability to assess and engage youth and families is critical.
• Physical ability to travel throughout the community to secure resources and services while adhering to the MST 120-day targeted length of treatment model.
• Willingness to adjust schedule as needed to meet service requirements.

Schedule

This is a full-time, hybrid/in-field position based in Rockaway, requiring home visits and travel to other locations.

Pay

$55,000.00 – $59,000.00 Annually
