Salary Range: $55,000.00 – $59,000.00 Annually

About the role

The MST Coach is a specialized Case Planner role with a concentration in the MST-FIT (Multi-systemic Therapy/Family Integrated Transition) model of service. Working within the social service department, the Coach supports youth and families, along with department staff, by wrapping services to ensure successful discharge.

Responsibilities

  • Perform case management using the MST approach, including all activities and administrative tasks associated with handling a caseload in compliance with ACS and OCFS regulations.
  • Provide aftercare support to cases transitioning to trial discharge using the MST-FIT model of treatment/therapy.
  • Offer additional support to cases transitioning from trial discharge MST-FIT to return to placement as necessary.
  • Provide crisis intervention and conflict resolution to youths and their families, including evenings and weekends on a consistent rotation schedule.
  • Conduct virtual and in-person sessions with resident youth and families.
  • Complete weekly documentation, including case, collateral, and other contacts within 48 hours of occurrence.
  • Ensure ongoing support and collaboration across MST and ITM program staff on behalf of residents.
  • Prepare and submit MST-FIT engagement court reports for scheduled permanency hearings.
  • Attend weekly and other ITM learning community meetings, trainings, and monthly mandatory trainings (including 7-day ITM and 5-day MST-FIT booster trainings).
  • Prepare for and attend weekly supervision, MST-FIT group consultations, and monthly MST-FIT psychiatric consultations as scheduled.
  • Support the social service department with resident transitions using the MST-FIT model of treatment engagement.
  • Perform fieldwork, including driving residents in your vehicle when required.

Requirements

  • Minimum MSW degree; licensure preferred.
  • Minimum of 1 year of experience working in child welfare with at-risk youth.
  • Former case management experience is a plus.
  • Valid NYS Driver’s License.
  • Professional, mature, and dependable with a bias for action.
  • Ability to assess and engage youth and families.
  • Physical ability to travel throughout the community to secure resources and services while adhering to the MST 120-day targeted length of treatment model.
  • Willingness to adjust schedule as needed to meet service requirements, including occasional evening hours for parent conferences and home visits.

Schedule

This is a full-time, hybrid/in-field position based in Rockaway, requiring home visits and travel to other locations.

Pay

$55,000.00 – $59,000.00 Annually
