Mental Health/ Care Companion Job Summary: Carease Health - IL is seeking a compassionate and dedicated Mental Health/ Care Companion to join our Collaborative Care team. As a key member of our mental health team, you will provide emotional support, brief therapeutic interventions, and mental health assessments to residents in skilled nursing facilities. Key Responsibilities: Visit designated facilities in accordance with the schedule set with your supervisorConduct assessments of residents to identify emotional, cognitive, and psychosocial needsImplement short-term, solution-focused therapy and supportive interventions to address immediate concernsDocument visit notes and maintain detailed records of interactions and interventions after each visitCollaborate with psychiatric professionals to ensure comprehensive care, including participation in a monthly meeting with the psychiatric NP Qualifications: Associates, Bachelor's, or Master's degree in Social Work, Psychology, Counseling, or another mental health–related fieldUnderstanding of geriatric mental health, crisis management, and psychosocial support in long-term care settingsStrong communication, documentation, and relationship-building skillsEducation: Associate DegreeExperience: 1 year Why Join Carease Health?
