Job Summary

Assists with access to care for CSNP members in collaboration with an RN case manager and nurse practitioner. Obtains information for members based on the model of care requirements. Obtains information to assist with evaluation of member needs. Communicate effectively with individuals across the care continuum. Documents member information in the electronic medical record. Reviews appointments and communicates pertinent information to the RN Case Manager(s) and nurse practitioner(s) for additional interventions. Collaborate with IDT for coordination of care Communicate with members and/or families obtaining and providing information regarding care and needs.

Job Requirements

  • Must be a high school graduate, or equivalent
  • Must be able to demonstrate computer skills, including use of e-mail, Microsoft Office Word and Excel.
  • Must be able to demonstrate effective written communication skills, including but not limited to, writing reports and correspondence.
  • Must be able to demonstrate effective verbal communication skills, including, but not limited to, speaking effectively to members, providers, families and staff and listening to members, providers and families
  • Demonstrated ability to meet deadlines, and use time effectively and efficiently.
  • Demonstrated successful experience working in an environment of change.
  • Effectively copes with change and uncertainty; responds and "shifts gears" comfortably
  • Dedicated to meeting the expectations and requirements of both external and internal customers
  • Demonstrated ability to establish and maintain effective relationships with customers, and gain their trust and respect
  • Uses logical methods to solve problems with effective solutions; looks beyond the obvious to solutions
  • Action oriented and sees challenges as opportunities.
  • Must meet company driving standards
  • Must have a valid state driver’s license and be able to drive a car
  • Must meet company's automobile liability insurance standards and provide proof of coverage
  • Must be able to independently travel to assigned locations
  • Knowledge of community resources
  • Previous experience working with behavioral health populations
  • Experience working with low income and disadvantaged populations
  • Previous work with vulnerable adults or senior population
