Responsibilities

  • Pass medications on assigned shift
  • Monitor medication administration to ensure accuracy, compliance, and resident safety
  • Conduct routine medication and treatment storage reviews in accordance with policy and procedure
  • Educate residents and caregivers on proper medication use, potential side effects, and adherence to treatment plans
  • Collaborate with healthcare providers, including physicians, pharmacists, and other nurses, to optimize medication therapies
  • Maintain accurate and detailed medication records and documentation
  • Other duties as assigned by Health Care Director

Requirements

  • Active Licensed Practical Nurse (LPN) license in Pennsylvania
  • Minimum of 2 years of clinical experience in a healthcare setting (focus on medication management preferred)
  • Strong understanding of pharmacology, medication administration, and drug interactions
  • Excellent communication and interpersonal skills
  • Ability to work independently and as part of a multidisciplinary team
  • Proficiency in electronic medication administration records (eMAR) systems

Preferred Qualifications

  • Experience in personal care, long-term care, hospice, or home health settings
  • Knowledge of regulatory standards and guidelines related to medication management
  • Commitment to continuous learning and professional development

This position may require working evenings and weekends as needed. Legend Senior Living is a drug-free workplace, and drug screening will be conducted at the company’s discretion.
