Responsibilities
- Pass medications on assigned shift
- Monitor medication administration to ensure accuracy, compliance, and resident safety
- Conduct routine medication and treatment storage reviews in accordance with policy and procedure
- Educate residents and caregivers on proper medication use, potential side effects, and adherence to treatment plans
- Collaborate with healthcare providers, including physicians, pharmacists, and other nurses, to optimize medication therapies
- Maintain accurate and detailed medication records and documentation
- Other duties as assigned by Health Care Director

Requirements
- Active Licensed Practical Nurse (LPN) license in Pennsylvania
- Minimum of 2 years of clinical experience in a healthcare setting (focus on medication management preferred)
- Strong understanding of pharmacology, medication administration, and drug interactions
- Excellent communication and interpersonal skills
- Ability to work independently and as part of a multidisciplinary team
- Proficiency in electronic medication administration records (eMAR) systems
Preferred Qualifications
- Experience in personal care, long-term care, hospice, or home health settings
- Knowledge of regulatory standards and guidelines related to medication management
- Commitment to continuous learning and professional development
