Responsibilities
- Interviews patients and family members to compile an accurate and complete list of current medications.
- Includes details such as name, formulation, dosage, route, PRN reasons, indications, last taken date and time, and information sources.
- Verifies patient allergy information and ensures complete documentation.
- Utilizes various resources for medication history collection, including patient interviews, pharmacy records, and outside facility records.
- Confirms medication compliance and documents discrepancies.
- Communicates complex/high-risk medications to the pharmacist.
- Assists pharmacists in preparing and distributing drugs, maintaining inventory, and managing records.
- Communicates with patients and family members regarding medication history.
- Determines medication list accuracy and collaborates with healthcare team members.
- Attends pharmacy staff meetings and reads departmental communications.
Qualifications
- High school diploma or equivalent.
- Two years of work experience in a retail pharmacy.
- Hospital pharmacy technician experience preferred.
- Registered pharmacy technician license in the state of practice required.
- Certified pharmacy technician preferred.
- ACPE accredited IV certification program, including hazardous (minimum 40 hours).
- BLS, ACLS, PALS for pediatric practice (optional).
- ASHP Medication History certification preferred.
