Responsibilities

  • Interviews patients and family members to compile an accurate and complete list of current medications.
  • Includes details such as name, formulation, dosage, route, PRN reasons, indications, last taken date and time, and information sources.
  • Verifies patient allergy information and ensures complete documentation.
  • Utilizes various resources for medication history collection, including patient interviews, pharmacy records, and outside facility records.
  • Confirms medication compliance and documents discrepancies.
  • Communicates complex/high-risk medications to the pharmacist.
  • Assists pharmacists in preparing and distributing drugs, maintaining inventory, and managing records.
  • Communicates with patients and family members regarding medication history.
  • Determines medication list accuracy and collaborates with healthcare team members.
  • Attends pharmacy staff meetings and reads departmental communications.

Qualifications

  • High school diploma or equivalent.
  • Two years of work experience in a retail pharmacy.
  • Hospital pharmacy technician experience preferred.
  • Registered pharmacy technician license in the state of practice required.
  • Certified pharmacy technician preferred.
  • ACPE accredited IV certification program, including hazardous (minimum 40 hours).
  • BLS, ACLS, PALS for pediatric practice (optional).
  • ASHP Medication History certification preferred.
