About the role

In Med History, our goal is to obtain the best possible medication history for each patient admitted to the hospital. This step is crucial to completion of Joint Commission accreditation-mandated medication reconciliation. Medication History Technicians claim patients from a work list based on the patient’s location and the tech's scheduled shift assignment. Technicians follow a structured interview with the patient or caregiver and document the information into the electronic medical record. Discrepancies are communicated directly to the provider/pharmacist for resolution.

Responsibilities

  • Provides technical assistance in the documentation and clarification of medication history lists under the direction of the pharmacist.
  • Processes, enters, and interprets physician/nurse medication histories, under the direction of the pharmacist, and completes full history documentation in computerized system.
  • Clarifies information through communication with providers, patients, and pharmacies.
  • Performs special projects for the department.
  • Works with multiple medication automation systems.
  • Provides professional customer service as a liaison between the pharmacy department and patients/caregivers.
  • Interacts with prescriber as needed.
  • Processes physician/nurse/pharmacist obtained patient histories and enters into the computer system and assists in maintaining patient profiles.
  • Reviews profile for allergies, duplication of drugs, and last doses and brings these to the attention of the pharmacist.
  • Checks and verifies accuracy of patient profiles.
  • Maintains clean and organized work area.
  • Maintains all requirements for CPhT certifications.
  • Completes all Corewell Health assigned competencies by assigned due dates.
  • Perform quality assurance data collection, collation, and presentation.

Requirements

  • High School Diploma or equivalent (required)
  • Associate's Degree in health care or related field (preferred)
  • 1 year of relevant experience in Pharmacy (required)
  • 2 years of relevant experience in Retail Pharmacy (preferred)
  • Pharmacist Intern License - State of Michigan (upon hire, required) or Pharmacy Technician License - State of Michigan (upon hire, required)
  • Pharmacy Technician Certification (CPHT) - PTCB (within 90 days, required) or Exam for the Certification of Pharmacy Technicians (ExCPT) - NHA (within 90 days, required)

Ideal candidate will possess Full Pharmacy Technician License and National Certification, such as PTCB or ExCPT, and 1+ year of experience as a Pharmacy Technician.

Schedule

  • On-Site position
  • 7 Days On / 7 Days Off
  • Night shift: 8 p.m. to 6:30 a.m.
  • Weekly Scheduled Hours: 36
  • Days Worked: Wednesday to Tuesday
  • Weekend Frequency: Every other weekend
